Your health record supports safe, continuous care when you see different clinicians. This page explains how to access your record and where to find further NHS guidance.
Access your records online
You can view parts of your GP record online using:
Online services can include information such as repeat prescriptions, test results, and consultation history depending on your access level.
If you need help with access
If you cannot access your records online, please contact the practice and we will help you.
- By phone: call reception.
- In person: speak to our reception team.
- By email or post: use details on our Contact page.
For some requests, we may need proof of identity to protect your confidentiality.

Opt out choices and data sharing
Your healthcare data can be used in different ways across the NHS to support your direct care, improve services, and support research and planning. You have choices about some types of sharing.
There are different opt-out routes depending on how data is used:
- Summary Care Record (SCR) preference: use the official NHS England Digital page to access the latest patient preference form (Word or ODT): SCR patient preference form. Return your completed form to the practice using our contact details (in person, by post, or by email).
- Type 1 opt-out (sharing from your GP record for planning and research): use the official NHS guidance page to access the latest Type 1 form and instructions: Opt out of sharing your health records. Return the completed Type 1 form to your GP practice.
- National Data Opt-out (NHS Digital and other organisations): Manage your choice online via NHS Your Data Matters.

Subject Access Request (SAR)
Much of your record may already be available online through the NHS App and NHS account. If you need a fuller copy of your records, you can make a formal Subject Access Request (SAR).

- Required ID: one form of photo ID (for example passport or driving licence) and one proof of current address (for example utility bill or bank statement).
- How to submit: send your request (using a form where provided) and ID details to the practice using our contact details.

Organ donation – recording your choice
Organ donation saves lives. In England, adults are usually considered for organ donation unless they opt out.
